Loop status — headline

Per-subject status of the four system-side automation stages that contribute to the overall verdict (Diagnose, Score, Decide, Act). Three additional stages are tracked separately because they're universally open today: Pre-test (mastered prereqs?), Diagnostic (XP-to-completion estimator), and Re-test (assigning a new test). Re-test is blocked by external-platform access; Pre-test and Diagnostic are unbuilt capabilities. None of the three are rolled into the overall verdict because doing so would collapse all variation across subjects.
